  • Bursal inflammation is often a secondary reaction to an underlying tendinopathy or mechanical overload.
  • A cortisone injection can reduce the bursa’s inflammation temporarily, but if the tendon is still being overloaded every time you walk, climb stairs, or sleep on your side, the pain returns.
  • Repeated injections can actually weaken tendon tissue over time, making the problem harder to fix long-term.

Common Root Causes We Find When an Injection Hasn’t Worked

  1. Gluteus medius and minimus weakness — Weak hip stabilizers force the IT band and gluteal tendons to overwork with every step, creating chronic irritation at the hip.
  2. Faulty gait mechanics — A hip drop (Trendelenburg pattern) during walking or running repeatedly compresses the tendons and bursa against the greater trochanter.
  3. IT band tightness and compression — A tight, overactive IT band increases friction and compressive load directly over the bursa.
  4. Referred pain from the low back or SI joint — Lumbar spine or sacroiliac dysfunction can mimic or worsen lateral hip pain, and an injection into the hip won’t touch a spine-driven problem.
  5. Core and pelvic instability — Poor core control changes how load transfers through the pelvis and hip during daily movement, increasing stress on the lateral hip structures.
  6. Leg length discrepancy or asymmetrical loading habits — Sitting with legs crossed, always sleeping on one side, or a true leg length difference can chronically overload one hip.
  7. Tendinopathy that was never addressed — If the gluteal tendon itself is degenerated, it needs a stimulus to actually heal and remodel — something an injection can’t provide.

If none of these root causes are identified and corrected, the pain — or the need for another injection — will almost always come back.
