Low Back Pain From Deadlifts? Here’s Why It’s Happening — And How to Get Back Under the Bar

If your low back has been barking every time you pull off the floor, you’re not alone — and you’re probably not as “broken” as it feels. Deadlift-related low back pain is one of the most common reasons lifters walk through our doors at Pursuit Physical Therapy, and it’s also one of the most fixable, if it’s treated the right way.

What’s Really Going On

Most lifters assume low back pain from deadlifts means a “blown disc” or permanent damage. In reality, the pain usually comes from one (or a combination) of a few things:

  • Breakdown in hip hinge mechanics — the low back starts doing work the hips and glutes should be doing, especially as the bar gets heavy or fatigue sets in
  • Disc irritation from repeated flexion under load, often from rounding at the bottom of the pull
  • SI joint or lumbar facet irritation from asymmetrical loading or bracing that breaks down mid-set
  • Muscle strain or guarding in the erectors and lumbar paraspinals, which can linger long after the “tweak” happens

The tricky part is that all of these can feel almost identical from the inside — a deep, achy tightness that flares with sitting, bending, or your next pull session. That’s exactly why guessing at the cause (or just resting and hoping) so often leads nowhere.

Why Rest and Generic Advice Fall Short

The typical advice — take a few weeks off, ice it, maybe grab a foam roller — treats the symptom, not the cause. Pain often calms down with rest, which feels like progress, right up until you load the bar again and it comes right back. That cycle is what keeps lifters stuck for months instead of weeks: rest, retest, re-injure, repeat.

Generic physical therapy can fall into the same trap. Cookie-cutter exercise sheets and 15-minute visits squeezed between three other patients aren’t built to find why your mechanics broke down under load in the first place — they’re built to get you through a plan of care.

The Pursuit Approach: Fix the Cause, Not Just the Pain

At Pursuit, every evaluation starts with figuring out exactly what’s driving your pain — not just where it hurts, but why it’s happening under a barbell specifically. That means looking at your hip hinge pattern, hip and core strength, bracing strategy, and how your spine is loading through a full pull, not just a generic low back screen.

From there, treatment is genuinely one-on-one, hands-on, and lift-specific:

  • Manual therapy and dry needling to calm irritated tissue, reduce guarding, and restore normal movement in the joints and muscles that are compensating
  • Hip hinge and bracing retraining so the pattern that caused the injury doesn’t just get rested — it gets corrected
  • Progressive loading, rebuilding strength and tolerance in the positions that matter, rather than avoiding them indefinitely
  • A structured return-to-lift plan that takes you from pain-free daily movement all the way back to pulling heavy, with benchmarks along the way instead of a vague “see how it feels”

Every session is one-on-one time with a Doctor of Physical Therapy — not a rotating cast of aides or a generic home exercise printout. That’s what a cash-based, concierge model allows us to do: no insurance caps dictating how much time you get or how many visits you’re “allowed” before you’re actually better.

Getting You Back Under the Bar — Not Just Off the Couch

Here’s where we think Pursuit’s approach really separates itself: our goal was never just “pain-free sitting at a desk.” For a lifter, success means pulling heavy again with confidence, not tiptoeing around your own back for the rest of your training life.

That’s the standard our team — including Ron Miller, DPT, OCS, Cert DN, and our doctors of physical therapy across both locations — holds every deadlift-related case to. We specialize in getting lifters real, measurable results: back to squatting, back to pulling, back to training the way you want to, not just “managing” symptoms indefinitely. We’re so confident in that process that our plans of care come backed by our results guarantee.
